| Event Activity Form | Please fill out this form for any event at Restoration Church whether it is held at the church or off site. Please fill out all required questions. (Any non-church event will be charged a $50 fee.) For weddings call the church office for price list. (Submit this form at least 1 month prior to the event.) |
